New Zealand Navy Ship Sank Last Month Due To ‘Human Error’

An inquiry has revealed that the grounding and sinking of the Royal New Zealand Navy vessel, HMNZS Manawanui, off Samoa’s coast on October 5 was caused by human error. The crew failed to disengage the ship’s autopilot, leading them to mistakenly believe a thruster control failure was responsible for the ship’s unresponsiveness. All 75 crew members survived the incident.
